Chart & Sign is sold by Balanced Nutrition With Kristen LLC ("we"). By unlocking or using the app you agree to these terms.

What it is

Chart & Sign is a writing aid for credentialed dietitians. It assembles draft documentation from phrases you choose and does routine arithmetic (weight change, BMI, estimated needs, enteral totals) from numbers you enter. It is not a medical device, does not make clinical decisions, does not connect to any health record system, and is not a substitute for professional judgment.

Your responsibility

You review, edit and approve every note before you chart it. You are responsible for checking every figure, formula value and statement, for the clinical content of what you sign, and for following your facility's policies and your professional standards.

License

One purchase is for one dietitian's use, on the computers that dietitian uses. Please don't share your key. The starter content may be changed freely for your own practice but not resold or redistributed.

No warranty

The app is provided "as is." We work to keep the math and built-in values accurate but do not warrant that they are free of errors or suited to every resident or facility.

Limitation of liability

To the fullest extent the law allows, we are not liable for any indirect or consequential damages arising from use of the app, and our total liability is limited to the amount you paid for it.

Refunds and support

Refunds follow the policy on the purchase page. Bug reports are welcome at balanced.nutrition.with.kristen@gmail.com; custom template building is not included.

Governing law

These terms are governed by the laws of the State of Texas.
